    ATTENTION 11TH GRADE PARENTS:

    • All 11th grade public school students are required to provide an 11th grade physical at some point during their 11th grade school year.  This obligation can be fulfilled with a sports physical, print out from the office visit or the school physical form found in the Student Health Forms.   
    • Please provide an updated immunization record

    ATTENTION 12TH GRADE PARENTS:

    • All 12th grade public school students must provide documentation of a Meningococcal A,C,Y, W booster vaccine (Meningitis).  This vaccine is typically provided at the student’s 16-year-old well visit.  This documentation MUST be received prior to the student entering 12th grade.

     All documents can be sent via submitted by taking a picture of the documentation and email to the nurses, student drop off in Nurse’s office or US mail.

    Student Emergency Forms
    A Student Emergency Form (pink) will be sent home with your student the first day of school.  Please complete all sections of the form including a parent signature and return the form by the first week of school to their homeroom teacher. 
    It is of the utmost importance that you complete these
    forms so that we, as a school system, are able to contact you with regards to your child. 
     
    Physical Education Excuses 
    If your child is ill or has a physical impediment that prevents him/her from participating in physical education a parental note is acceptable for one class period following the injury.  If your child needs to extend that excuse for a longer period of time you must provide the school with a physician's note with the diagnosis and length of time that the child will be unable to participate in physical education.
     
    Medication
    As per District Policy, students are not to bring any medication to school without a physician's order and parent note.
    Over -the counter medications listed on the Student Emergency Form are administerd with permission from the school physician and parent approval as indicated by parent signature and checking "YES" on the medication. Any other over-the-counter medication will require a physician and parent note.

    Early Release Due to Medical Concerns:
    If your child becomes ill/injured during the school day this procedure will be followed:
     
    1.  The student is to report to the nurse's office to be assessed for dismissal from school.  Student's are not permitted to contact their parent prior to being assessed.
    2.  The Parent/guardian will be called first. A student will not be released to anyone that is not listed on the emergency contact list unless, the parent calls and states name of the individual that will be picking up student from school.
    3.  Students will sign out in the attendance office. Parents may be asked to come into school and sign out the student.

    MEDICATION POLICY

    ALL PRESCRIPTIVE, OVER-THE-COUNTER (OTC) & HOMEOPATHIC PREPARATIONS ARE TO BE GIVEN AT SCHOOL BY THE SCHOOL NURSE

     

    1. A SIGNED PHYSICIAN’S NOTE IS TO BE ON FILE FOR EACH MEDICATION.
    2. A SIGNED PARENT/GUARDIAN NOTE IS TO BE ON FILE FOR EACH MEDICATION.
    3. THE MEDICATION MUST BE IN THE ORIGINAL CONTAINER.

    Notes expire at the end of the school year and must be renewed for the following school year.

    The school district will provide the following OTC medications with permission to dispense by the school physician:  Acetaminophen (Tylenol),  Ibuprofen (Advil),  Mylanta,  Pepto Bismol, Benadryl,  Coke Syrup, Throat spray/lozenge, Neosporin Ointment

    Please check those to be given on the Student Emergency Form.

    Students are NOT permitted to carry medication in school.  This includes all prescriptive, OTC and homeopathic preparations. Violation of this policy will result in disciplinary action which may include recommendation for expulsion.

    The school physician may be consulted by the school nurse at any time with concerns regarding medications.

    INHALER USE AT SCHOOL

    Act 187 of the Pennsylvania School Code allows students to possess and self-administer an asthma inhaler in the school setting under certain guidelines.

    STUDENTS who desire to possess and self-administer an asthma inhaler will be required to:

    1. demonstrate that he/she is capable and responsible to self-administer the inhaler
    2. notify the school nurse immediately following each use of the asthma inhaler
    3. not allow other students to use the inhaler         

    Failure to follow the policy will result in the immediate confiscation of the inhaler and loss of the privilege of possessing the inhaler for self-administration.

     

    PARENTS will provide written permission to include:

    1. they agree with the physician order and the school to allow their child to possess and self-administer the asthma inhaler
    2. to relieve the school and school employee of any responsibility for the benefit or consequences of the prescribed medication
    3. the school bears no responsibility for ensuring that the medication is taken

    PHYSICIANS will provide a written permission to include:

    1. name of the drug, dose, time to be administered and the diagnosis or reason for the medication
    2. indicate any reaction that may occur to the medication and any emergency response
    3. state whether the student is qualified and able to self-administer the medication

    Permission to possess and self administer must be renewed yearly. 

    An Asthma Action Plan should be completed for each student with asthma.

    It is recommended that students see their personal physician but exams can be done at school by the school physician, Dr. Jeffery Brand. Permission forms, with parent signature are required prior to the examination.  Please make a copy of the sports physical for the school nurse and include an immunization update.  The State Private Physical Form can be found here.

    DENTAL EXAMINATION SCREENING

    Grade K or 1, 3, 7

    It is recommended that the student see their private dentist but exams can be done at school by the school dentist, Dr. Sheldon Boruchow. Permission forms, with parent signature are required prior to examination. The State Private Dental Form can be found here.
